 The 3-hab model is quite different regarding the management of disability injuries. We recognized early on that effective case and utilization management required a specialized staff who understood medical providers. 3-hab's specialized physician case managers work collaboratively with medical providers to achieve a common goal of returning disabled workers back on the job safely and timely.


David Greenfield, M.D.

Our Founder, CEO, and Senior Clinician


Dr. Greenfield, practiced Orthopedic Surgery for over 30 years. He identified problems that existed with his own patients becoming deconditioned when an employer wouldn't accommodate his work restrictions thus paving the way for 3-hab. Under his guidance 3-hab has positively impacted the quality of life for many disabled workers, and made a stronger bottom line for numerous employers since 1994.



Lee D. Shaftel, M.D.

Medical Director 


Dr. Shaftel, specializes in multiple areas of Orthopaedic care. He completed his internship and residency in General and Orthopaedic Surgery at the University of Cincinnati Medical Center. He received his M.D. from the Chicago Medical School/University of Health Services. He received his pre-med degree from Syracuse University. He is certified in Orthopaedic surgery by the American Board of Orthopaedic Surgery. He currently practices at Christ Hospital.



Richard A. Jolson, M.D.

Associate Medical Director 


 Dr. Jolson, a retired Orthopedic Surgeon and Sports Medicine Physician for over 40 years, is one of our Orthopedic Case Managers. He is a graduate of the University of Cincinnati. He was once the team physician for the Cincinnati Reds. His communication skills and vast understanding of Orthopedic problems, makes the collaboration with the Physician's of Record a win/win for all parties to the claim.



Nicholas Jones, D.C.

Associate Medical Director


 Dr. Jones, has been practicing chiropractic care for over 15 years. His specialties are in Occupational Health and Forensics. He graduated from Northwestern College of Chiropractors. He currently practices in an office in Greater Cincinnati. His management excels at holding his peers to their own chiropractic standard.



Bernard Bacevich, M.D.

Associate Medical Director 


 Dr. Bacevich,  is a graduate of the University of Cincinnati medical school. He completed his orthopaedic training at Thomas Jefferson University in Philadelphia, PA. He is a board certified member of the American Board of Orthopaedic Surgery. He practiced general orthopaedic surgery for 29 year before retiring in the greater Cincinnati area. He has a special interest in the evaluation and treatment of personal and work related injuries.  



James B. Willis, M.D.

Associate Medical Director 


Dr. Willis, is a graduate of the University of Michigan College of Medicine. He completed his internship at Sinai Hospital of Detroit, and his residency in General Orthopaedic Surgery at the University of Cincinnati Medical School. He also completed a fellowship in Hand Surgery at the University of Louisville. He is certified in Orthopaedic surgery by the American Board of Orthopaedic Surgery. He currently practices at Mercy Hospital.



David Schwartz, Ph.D.

Associate Medical Director


Dr. David Schwartz, earned his B.S. in Psychology from the University of Wisconsin.He earned his Ph.D. in Clinical Psychology from Vanderbilt University, specializing in behavioral medicine and Health Psychology. He was an intern and post-doctoral fellow at the University of Virginia Medical Center. He has had medical school appointments at the University of Virginia, Vanderbilt University, and the University of Cincinnati. He has published and presented extensively in pain management and behavioral medicine. He has been in private practice since 1995 specializing in complex medically involved patients. He is currently a partner in Hamilton Health Associates, a multi-specialty practice focused on industrial injury.   He is a consultant to the Freiberg Spine Institute and a member of the Disability Evaluation Panel for the Ohio Bureau of Worker’s Compensation (BWC). He presently serves as the Ohio Psychological Association and Ohio Board of Psychology’s representative on the Ohio Bureau of Workers Compensation Health Care Quality Assurance Advisory Committee. He chairs the Ohio Psychological Association Task Force on Workers Compensation Reform.



Anne Like, MD

Associate Medical Director


Dr. Like, is a graduate of the University Of Cincinnati College Of Medicine. She completed her internship at Jewish Hospital and completed her Occupational Medicine Fellowship at the University of Cincinnati. She holds Board Certification in Internal Medicine as well as Prevenitive Occupational Medicine. She has practiced in the Cincinnati area since 1995. She currently holds the position of Director of Employee Health and Disability Management at Christ Hospital in Cincinnati. Her experience in industrial disability, corporate disability, and wellness and prevention has empowered her as a valued member of the physician case manager team at 3-Hab.  


Aaron R. Shaftel, DDS

Associate Medical Director


Dr. Shaftel, received his doctorate of dental surgery from The Ohio State University College Of Dentistry. He completed his AEGD residency at the University Of Cincinnati Medical Center. His private practice located in West Chester, Ohio, focuses on implant and comprehensive dentistry.
